Community Integration Instructor
Walnut Creek; Tri-Valley Area

Purpose: To support adults with developmental disabilities on-the-job and in community activities.

Reports to: Community Integration Coordinator

Qualifications:

  • Previous experience working with target population or other relevant experience, preferably in a community-based service.
  • Knowledge of American Sign Language (ASL) desired not required, ability to communicate with minimal-language and foreign-born deaf adults.
  • First Aid certification.
  • Ability to work flexible hours may be needed.
  • Health screening, TB test and fingerprinting as required by the State Department of Developmental Services.

Responsibilities

  • Provide communication skills training, including American Sign Language and reading and writing skills in English.
  • Provide instruction and support in areas that enable clients to access the communities of their choice, including, but not limited to: public transportation, travel training, employment, education, recreation and leisure skills, hygiene, budgeting, and safety awareness.
  • Provide instruction regarding client rights and self-advocacy.
  • Responsible for maintaining client progress data and evaluating client skills
  • Responsible for completing all required documentation, including monthly billing reports, case notes and additional case records.
  • Attend all required meetings and in-services.
  • Responsible for maintaining a professional relationship with referral sources, employers, parents, families and appropriate others.
  • Responsible for maintaining confidentiality.
  • Ability to travel reliably in the East Bay.
  • Other reasonable duties as assigned by supervisor.

Community Support Facilitator
San Francisco Bay Area

To coordinate services and provide support to adults with developmental disabilities in maintaining independent living.

Responsibilities
  • Provide instruction and support around all areas of daily living, including, but not limited to: meal preparation, shopping, hygiene, personal care, budgeting, safety awareness, and maintaining a clean and healthy living environment.

  • Provide case coordination- make referrals, monitor client progress, complete all required paperwork.

  • Adapt communications to meet the needs of people served.

  • Supervise agency staff, as assigned. Assist clients in choosing and supervising their outside agency staff.

  • Provide instruction regarding client rights and self-advocacy.

  • Provide training and support in areas that enable clients to maintain their own homes in the community of their choice.

  • Responsible for maintaining professional conduct with clients.

Qualifications

  • A minimum of six months experience working with adults with developmental disabilities and/or a willingness to work with developmentally disabled adults and other relevant experience.
  • Supervisory experience preferred.
  • First Aid/CPR certification.
  • Willingness to work flexible hours to meet client needs.
  • Computer proficiency preferred, along with initiative and the ability to think independently as well as work in a team.
  • Takes initiative and thinks independently while maintain a spirit of cooperation and collaboration.
  • Must be creative, flexible and well organized with good planning abilities.
  • Must also possess excellent people skills and communication skills. Successful candidates should also be able to interact effectively with a diverse age group and population.

Instructor - On-call in the East Bay, Community Support (.25 FTE Position)

This position has a focus on supporting individuals with disabilities (living in supportive housing) in developing vocational and independent living skills. Qualifications: a minimum of one year's experience in teaching and supervising adults with disabilities, preferably in a vocational setting. Knowledge of issues facing people who are formerly homeless, people with substance abuse disabilities, and people with mental health disabilities. Knowledge of service providers, community resources, and the vocational rehabilitation system. Excellent written and communication skills, and ability to work flexible hours to meet client needs. Bilingual capabilities strongly preferred. Starting salary $12.75 an hour.

Janitorial Supervisor - Full-Time in San Francisco

To provide supervision and training of client workers as well as perform janitorial work.

Essential Functions :
  • Provide supervision and training of janitors in janitorial skills.
  • Perform janitorial work, as needed.
  • Perform and document quality control inspections of project area, customer satisfaction surveys and records of janitor trainings.
  • Participate in, and implement, short and long range planning activities.
  • Order, maintain and monitor supplies and equipment.
  • Work directly with Customer representative to fulfill contractual obligations and resolve problems.
  • Responsible for maintaining a professional relationship with Customer staff, tenants, Gevernment representatives and Company staff.
  • Perform other related duties upon request of Toolworks’ management.
Qualifications
  • Ability to work well with adults with disabilities.
  • Experience in janitorial work/supervision.
  • Ability to clearly demonstrate janitorial skills to others.
  • Ability to work as a member of a team.
  • Ability to speak Cantonese highly desirable.
  • Possess verbal and/or written English skills.
  • Able to pass security clearance.
  • Able to lift 35 pounds.

Job Developer - Regular Full-Time, Employment Services

The Job Developer provides an array of employment opportunities for people with developmental disabilities; reports to the Director of Employment Services.

Job Duties Include:
  • Develop and identify opportunities to meet the specific needs and interests of individual clients.
  • Prepare and perform client assessment; develop individual plans for employment.
  • Consult with clients and service providers to develop array of vocational activities and employment opportunities.
  • Outreach to community to expand employer and community partnerships.
  • Participate in various community and Toolworks' meetings related to job development.
  • Act as contact and liaison with GGRC, Department of Rehabilitation and other referral sources.
  • Maintain contact with employer and client following placement to ensure job stability.
  • Responsible for reviewing case records and ensuring appropriate services are being delivered to all individual placement clients.
  • Responsible for completing monthly billing paperwork for services, as well as Habilitation and Department of Rehabilitation, when appropriate.
  • Participate in annual client IPP meetings.
Qualifications
  • Past experience with people with developmental disabilities and/or with job placement.
  • Awareness of different employment models, such as micro-enterprise, direct placement, customized employment, etc.
  • Ability to research local labor market trends.
  • Bilingual abilities strongly preferred (American Sign Language or Spanish).
