Community Support Facilitator
San Francisco Bay Area

The ideal candidate will be able to develop business opportunities leading to employment for persons with disabilities in the East Bay. He/She should also come with energy, enthusiasm, and be highly motivated!

Responsibilities
  • Provide instruction and support around all areas of daily living, including, but not limited to: meal preparation, shopping, hygiene, personal care, budgeting, safety awareness, and maintaining a clean and healthy living environment.

  • Provide case coordination- make referrals, monitor client progress, complete all required paperwork.

  • Adapt communications to meet the needs of people served.

  • Supervise agency staff, as assigned. Assist clients in choosing and supervising their outside agency staff.

  • Provide instruction regarding client rights and self-advocacy.

  • Provide training and support in areas that enable clients to maintain their own homes in the community of their choice.

  • Responsible for maintaining professional conduct with clients.

Qualifications

  • A minimum of six months experience working with adults with developmental disabilities and/or a willingness to work with developmentally disabled adults and other relevant experience.
  • Supervisory experience preferred.
  • First Aid/CPR certification.
  • Willingness to work flexible hours to meet client needs.
  • Computer proficiency preferred, along with initiative and the ability to think independently as well as work in a team.
  • Takes initiative and thinks independently while maintain a spirit of cooperation and collaboration.
  • Must be creative, flexible and well organized with good planning abilities.
  • Must also possess excellent people skills and communication skills. Successful candidates should also be able to interact effectively with a diverse age group and population.

Coordinator of Community Integration
San Francisco Bay Area

To assist the Assistant Director of Community Integration in managing the day to day operations of Community Integration Services.

Responsibilities
  • B.A. in Special Education, Rehabilitation Counseling or related field preferred; or a minimum of three year's experience working with adults with developmental disabilities required, preferably in a community setting. Two years supervisory or program management experience preferred.

  • American Sign Language skills desired not required, with ability to work in a bilingual/bicultural environment with deaf and hearing clients and staff.

  • First Aid/CPR certification.

  • Willingness to work flexible hours to meet client needs.

  • Excellent report writing skills and the ability to complete all required program paperwork.

  • Microsoft office proficient. Computer skills required.

Qualifications

  • Supervise, train and evaluate staff and assist with staff development, as delegated by the Assistant Director of Community Integration.

  • Coordinate daily operations for the site.

  • Coordinate client and staff schedules for the site.

  • Serve as agency liaison role with community members, employers, social service agencies, and client families.

  • Responsible for person centered planning and developing Individual Support Plans.

  • Teaching client rights and developing self-advocacy skills.

  • Assist staff in helping clients gain the skills necessary to increase their independence in the community. Provide instruction and support in areas that enable clients to access the communities of their choice, including, but not limited to: public transportation, travel training, employment, education, recreation and leisure skills, hygiene, budgeting, and safety awareness.

  • Responsible for maintaining professional conduct with clients and in maintaining confidentiality.

  • Attend all required meetings.

  • Responsible for maintaining client progress data and evaluating client skills.

  • Responsible for completing all required documentation, including monthly billing reports, case notes and additional case records and ensure staff are completing all required paperwork and case files are complete.

  • Perform direct care duties as needed.

  • Responsible for Resource Development within the community

Employment Outreach
East Bay Area

The ideal candidate will be able to develop business opportunities leading to employment for persons with disabilities in the East Bay. He/She should also come with energy, enthusiasm, and be highly motivated!

Responsibilities
  • Identify and develop Bay Area employer relationships through networking and other marketing that leads directly to the employment for persons with deafness and psychiatric disabilities
  • Ability to access, analyze and apply local labor market information leading to new employment opportunities
  • Work closely with job seekers and other staff to identify client's interests and skills
  • Maintain database and complete progress reports and other required paperwork

Qualifications

  • Previous experience in job development, sales and/or marketing required
  • Excellent writing and verbal communication skills
  • Able to create and make presentations to business and community leaders
  • Excellent computer skills with proficiency in MS Office Suite
  • Previous experience working with persons with disabilities and familiarity with persons who are deaf (ASL) and/or psychiatric disabilities desirable
  • Ability to travel throughout East Bay
  • Able to work flexible hours when needed

Instructor - On-Call, Community Integration

Positions support adults with developmental disabilities in on-the-job and in community activities. Qualifications: Previous experience working with target population or other relevant experience, preferably in a community-based service. Knowledge of American Sign Language preferred. First Aid certification. Ability to work flexible hours may be needed. Health screening, TB test and fingerprinting as required by the State Department of Developmental Services.

Instructor - On-call in the East Bay, Community Support (.25 FTE Position)

This position has a focus on supporting individuals with disabilities (living in supportive housing) in developing vocational and independent living skills. Qualifications: a minimum of one year's experience in teaching and supervising adults with disabilities, preferably in a vocational setting. Knowledge of issues facing people who are formerly homeless, people with substance abuse disabilities, and people with mental health disabilities. Knowledge of service providers, community resources, and the vocational rehabilitation system. Excellent written and communication skills, and ability to work flexible hours to meet client needs. Bilingual capabilities strongly preferred. Starting salary $13.90 an hour.

Job Coaches - On-Call, Employment Services

Committed, flexible professionals sought to provide on-call supervision and training of crews of adults with disabilities, and address vocational skills and appropriate work habits. Previous experience in teaching & supervising adults with disabilities in a vocational setting required. American Sign Language required. Ability to work flexible hours required. Starting salary range $12.01 to $12.98 an hour D.O.E.
