Toolworks, in partnership with people with disabilities, is a human service agency dedicated to providing the tools and resources that promote independence, equality, and personal satisfaction. Toolworks provides a number of programs including: job placement and training, community integration, and supported living services.
Position Title: Community Living Facilitator-ILS
Reports to: Assistant Director Community Living-ILS
Supervises: N/A
Status/level: Non-exempt
Purpose: To support people with disabilities in maintaining independent community living.
Starting Date: Immediately
Duties and Responsibilities:
Client Support Functions
- Provide attendant care, in-home support, and training to clients in all areas of daily living, including, but not limited to: meal preparation, shopping, hygiene, personal care, budgeting, safety awareness, and maintaining a clean and healthy living environment
- Assess client interests and abilities through the person-centered planning process, develop written plans.
- Contributes to the development of individual support plans
- Assist clients in managing personal affairs
- Provide linkages in the community for needed services; provide case management and service coordination
- Creates and uses natural supports and supports client in a person centered manner
- Recognize unusual or threatening conditions related to persons with disabilities and resolve issues if necessary
Administrative Functions
- Documents client progress in T-logs, Responsible for completing all required documentation, including monthly billing reports, case notes and additional case records
- Creates and maintains schedule based on client need and authorized hours
- Completes all other required paperwork
- Responsible for maintaining professional relationships with clients, referral sources, employers, families, care providers and appropriate others; and for maintaining confidentiality
Organizational (all staff)
- Participate in staff meetings, trainings and retreats as required
- Comply with current agency performance standards
- Report changes in criminal or employment status on a timely basis
- Represent the organization to visitors and the community as needed
- Maintain a professional attitude, working cooperatively with other staff, clients and members of the community
- Maintain confidentiality of client/staff information as required by law and agency policy
- Treat clients and co-workers with dignity and respect

Skills, Knowledge and Abilities:
Education and Experience Requirement:
- Minimum of two years’ experience working with adults with disabilities required, preferably in a community setting. May substitute education in a related field for experience
Skills requirement:
- American Sign Language, Spanish or Cantonese skills preferred. Some level of fluency for certain target populations may be required.
- Fluent report writing skills
- Willingness to work flexible hours to meet client needs.
- Able to recognize unusual or threatening conditions related to persons with disabilities
- Strong time management skills, patient, detail oriented
- Proficient in Microsoft Office Suite
Program requirements:
- Fingerprint background check (Child Abuse Index, Criminal Record Clearance)
- First Aid/CPR certification required (within 30 days)
- Clean DMV printout and proof of auto insurance

- $18/hr
